결 론
뇌졸중은 만성적인 건강문제이므로 지속적인 재활간호가 요구되기 때문에 이들을 간호하기
위한 가정간호 프로토콜의 개발이 필요하다. 그러므로 본 연구에서는 체계적인 가정간호 프
로토콜을 개발하여 적용효과를 검증하고자 하였다. 연구목적을 달성하기 위하여 1997년 10
월부터 약 5개월간 가정 간호사 13명과 재가 뇌졸중환자 30명을 대상으로 단일군 사전사후
설계로 프로토콜 적용전과 후의 간호사의 업무만족도, 환자의 만족도와 일상생활동작 회복
정도를 측정하였다. 수집된 자료는 Wilcoxon signed Ranks test와 paired t -test로 분석하
였으며 그 결과는 다음과 같다.
1. 가정간호 프로토콜을 적용하기전보다 적용한 후의 간호사 업무만족도가 높아(t=-2.814,
P=.005) 제 1가설은 지지되었다.
2. 가정간호 프로토콜을 적용하기전보다 적용한 후의 환자의 만족도가 높았으나, 통계적으
로는 유의한 차이를 나타내지 않아(t=-.416, p=.680) 제2가설은 기각되었다.
3. 가정간호 프로토콜을 적용하기전보다 적용한 후의 환자의 일상생활동작 회복정도가 높
아(t=-3.881, p=.001) 제3가설은 지지되었다.
이상의 결과를 요약하면 환자간호시 가정간호 프로토콜을 사용하면 간호사의 업무만족도도
높아지고 환자의 만족도는 통계적으로 유의하지는 않지만 높아졌다. 또한 일상생활동작 회
복정도도 향상되어 가정간호 프로토콜의 긍정적인 효과가 입증되었다 그러므로 가정간호를
위해 질병별 가정간호 프로토콜 개발이 요구되며 특히 가정간호사 경력이 적거나 신규근무
일 경우 구체적이고 체계적인 프로토콜이 더욱 필요하다 이는 궁극적으로 환자의 질적 간호
제공에 도움이 되리라고 본다.
Since patients who have suffered from strokes have chronic health problems, the
development of a protocol for home care nursing is required. Therefore, this study was
done to test the effects of such a protocol on stroke patients.
This study was conducted by using a single group pre -post design. The subjects for
the study were 13 home care nurses and 30 home care patients who had a stroke.
Data collection was done from October 1997 to February 1998.
The collected data was analyzed using the Wilcoxon signed rank test and paired t-test
and the results were as follows :
1. The first hypothesis that "working satisfaction of home nurses would be higher after
using the home care protocol" was supported.
2. The second hypothesis that "patients' satisfaction would be higher after using the
home care protocol" was dejected.
3. The third hypothesis that "the level of ADL of patients would be higher after using
the home care protocol" was supported.
In conclusion, by using a home care protocol, the satisfaction of nurses' work and the
ADL of patients increased, and the patients' satisfaction also increased without
statistical significance.
The above results confirm that the application of a home care protocol Is very
effective.
Thus, this protocol can be helpful in the quality care of stroke patients.
Therefore, developing and organizing protocols, according to health problem, can be
useful for beginner nurses who interact with high risk patients.
